Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.
Purpose: The HERO Case Manager plays a critical role in supporting a coordinated, client-centered recovery environment by integrating clinical, operational, and community-based services. Working collaboratively across interdisciplinary teams, this position ensures safety, accountability, and continuity of care for individuals navigating co-occurring substance use and mental health recovery. The role focuses on connecting clients to appropriate referrals and resources that support both immediate needs and long-term recovery, while partnering with clinical staff, leadership, and administrative teams to maintain a safe and structured program environment. Through ongoing assessment, the Case Manager identifies client needs related to physical and mental health, housing, employment, family support, and other essential services, and works closely with the HERO care team and community partners to implement individualized solutions. This position supports clients in preparing for independent living while ensuring all services, client engagement, and outcomes are accurately and timely documented in alignment with program standards, compliance requirements, and overall quality of care. |

Key Accountabilities: ·Maintain a safe, structured, and therapeutic environment by consistently following all safety, security, and program guidelines, including class participation, behavior compliance, and client activity as needed. ·Facilitate and coordinate daily recovery programming, including groups, meetings, and structured activities aligned with the HERO continuum of care (STR, IOP, OP). ·Complete comprehensive intakes and client orientations, ensuring everyone is welcomed into the program with clear expectations, resources, and support. ·Develop individualized service plans within 24 hours of admission, in collaboration with clinical staff and leadership, ensuring plans are person-centered, goal-oriented, and responsive to client needs. ·Implement and manage service plans through ongoing engagement with clients, interdisciplinary teams, and community providers; documenting services, progress, barriers, and outcomes in a compliant manner. ·Ensure all documentation is entered into the Electronic Health Record within 72 hours of service delivery, maintaining accuracy, compliance, and alignment with agency and regulatory standards. ·Coordinate access to medical, dental, psychiatric, and community-based services, strengthening partnerships to support whole-person care and successful transitions. ·Support high-acuity and dual-diagnosis clients’ coordination of care to reduce attrition and enhance retention. ·Assist clients in navigating systems and completing applications related to healthcare, income, housing, and other supports necessary for successful community reintegration. ·Participate actively in interdisciplinary team meetings, contributing to case consultations, care planning, and shared problem-solving to ensure continuity and quality of care. ·Support program operations through daily responsibilities including room checks, chore verification, medication observation, and reinforcement of program expectations and client accountability. ·Demonstrate strong communication, teamwork, and interpersonal skills, contributing to a positive, professional, and solution-focused team culture. ·Demonstrates strong organizational and time management skills to effectively manage multiple responsibilities in a fast-paced environment, balancing immediate client needs with long-term goals and outcomes. ·Uphold strict confidentiality and ethical standards in all aspects of client care and program operations. ·Provide support to program leadership as needed and contribute to ongoing program development excellence. ·Ensure all safety and security guidelines are followed. ·Coordination of all Recovery activities and meetings. ·Proficiency with Microsoft Office and excellent written, oral and interpersonal skills. ·Other duties as assigned.
